MEDICAL RECORDS
The forms that are required upon entry to the school.
All students under 18 years of age must show proof of compliance with the immunization requirements of NYS PUBLIC HEALTH LAW,SECTION 2164.
Saint Vincent Ferrer High School Policy require that the medical forms be submitted upon entry to the school.
Failure to provide the school with completed immunization records will result in the student's inability to enter school in September.
Free testing and care is available at several NYC clinics located in Manhattan,Bronx,Queens and Brooklyn.
